O ne of the Hanafi readers provided the follow (may Allah reward him):
Al-Salam `Alaykum

[wa `alaykum al-salam wa rahmatullahi wa barakatuhu]

A few notes on the Hanafi Qunut:
 
... in the third after reading al-Fatiha and a Sura.
...the du`a after the takbir is made softly by both Imam and Muqtadi.
